CHILD SUPPORT PROCEDURES
An initial appointment will
be scheduled for you to meet with an Intake
Officer at the Benton Courthouse, District
Attorney's Office, Benton, LA. The purpose
of this appointment will be to secure additional
information from you in an effort to secure
child support and/or medical support on behalf
of your minor child(ren).
If you are receiving cash assistance
or Medicaid, your failure to cooperate will
result in your case being closed. If you are
not receiving cash assistance or Medicaid,
your failure to keep this appointment may
result in your child support case being closed.

If you cannot keep this appointment, please notify
this office immediately.
Bring this information with you to the appointment,
if available:
*Name of the employer of the absent parent
*Home or employment address of the absent parent
*Your social security number and the social security
number(s) of your children
*The absent parent's social security number
*Marriage license
*Certified divorce/separation documents and the
child support orders
*Birth Certificate of the Children
*Last (3) three paycheck stubs
If you were not married to the father of your child(ren),
please bring the following, if available:
*Photographs of the father with the child(ren)
*Letters or cards from the father mentioning or making
reference to the child(ren)
*Names and addresses of persons who have knowledge of
the alleged father's relationship with the child(ren)
*Names and addresses of anyone who has knowledge of
your relationship with the father.
